A future near miss report can be more accurately (and more quickly) completed if, given a familiarity with the questionnaire, you are able to mentally rehearse your answers immediately after an actual near miss. Important: You'll first need to remember your willingness to participate (when a near miss occurs), thus please sign up for the newsletter and receive reminders with a frequency of your choice! A few things you need to know before proceeding with this preview... The only difference between this preview and the actual questionniare is that responses are not saved in the database. Most of the questions can be previewed by simply clicking the "NEXT" button on each page. Note that your near-miss reports will likely NOT require answering all the questions shown in this preview. You'll be able to see some questions only when a pop-up window is triggered by a particular multiple-choice selection, so a little experimentation may be in order!
